Adult Learning Disability Community Health Team


We are a Specialist Community Health Team offering a service to people over the age of 18 who have a learning disability and require specialist health care.

Eligibility criteria:
Our criteria is for adults over the age of 18 who have a Learning Disability, as defined in Valuing People Now which is defined as having an IQ of 70 or below.
Provides support to:
  • People with SEN/learning disabilities
Is a referral required?:
Accepted referral methods:
  • Self referral
  • Via CAMHS (Child and Adolescent Mental Health Services)
  • Via GP or Health Professional
  • Via Jobcentre Plus
  • Via College or post-16 provider
  • Via Children’s Social Care
  • Via Adult Social Care
  • Via Cheshire East Housing Options Team
  • Via Voluntary Agencies
  • Via Youth Offending Team and/or Youth Prevention Team
  • Via Youth Support Service
Additional Referral Information:
Referral made to the Team and discussed weekly at the allocations meeting each Monday morning. Once allocated identified staff member will make contact. Urgent referrals will have contact made within a week, routine referrals are usually within two weeks. This will depend on the number of referrals received, some sections of the team may have a waiting list due to capacity levels.
