Care Act Advocacy

Description

The Care Act advocacy service works to help individuals aged 16 and over and their carers who have a substantial difficulty in engaging in the social care assessment and planning process. Care Act advocates can support an individual with assessments, care and support planning, care reviews, safeguarding enquiries and to submit appeals.

In essence an advocate represents a sort of unconditional ally for a vulnerable individual, they may in fact be the only person in that individual’s life who is on their side. Our Advocacy services are free, confidential and independent. Our services can be provided following a referral from a health or social worker professional, if deemed necessary, following an assessment of your needs. Cheshire Advocacy Hub provides the following advocacy services: 
 
• Independent Mental Health Advocacy 
• Independent Mental Capacity Advocacy, DOLS & paid RPR 
• Independent Advocacy under the Care Act 2014
