Get Yourself Active Project

To support disabled people and people with long term health conditions to access the sport and leisure opportunities of their choice, within their community. The Get Yourself Active project acts as a signposting and information service for people who are interested in accessing sport and physical activity. Once a referral has been made, the coordinator will contact the family to arrange a home visit or to provide information on activities via email or post.
