16 - 19 Mental Health Team

Description

We are going to be working in partnership with Visyon from October of this year. This will hopefully enable us to make our service more accessible and provide more options for engagement with services. We work with specific Mental Health needs and work collaboratively with the young people that access our service treating them as young adults working toward improving well being, independence, robustness and resilience. If required we support transition into Adult Services.

Eligibility criteria:
Must have a Mental Health need and an available intervention that the young person is prepared to engage with.

  • Via CAMHS (Child and Adolescent Mental Health Services)
  • Via GP or Health Professional
  • Via School
  • Via College or post-16 provider
  • Via Children’s Social Care

  • Via Voluntary Agencies
  • Via Youth Offending Team and/or Youth Prevention Team
  • Via Youth Support Service
Waiting List:
We are contracted to see 200 new referrals per financial year (East only). We are not an urgent response service although we do support services that do see our young people in crisis by consultation. We usually have a wait of 4-6 weeks if the referral is felt appropriate for our service. This wait time would be longer in Crewe areas as the team is commissioned differently by the South CCG.
